Please note that the vaccinations you’ll need depend on the specific location you’re traveling to. Our experienced practice nurse will guide you through the necessary vaccinations based on your travel plans.
Please contact the surgery to book an appointment before you fill in the travel questionnaire. You can find the questionnaire by scrolling down on this page. Please fill in all requested information, including your full vaccination history with date of last vaccination. Failure to complete this information may prevent you from having the vaccine.

Failure to submit the completed questionnaire at least one week before your appointment will result in automatic cancellation of this appointment.
